Checklist for Data Quality Assurance of FP Service Data

This is a tool developed by the Nigerian Urban Reproductive Health Initiative to access the level of data quality at every NURHI facility.  It contains three distinct features: Availability of data collection materials; Accuracy of data; and Consistency of data collected in every facility visited.
